Your Rights

At Bonneville Surgery Center, we are committed to ensuring you receive respectful, high-quality care. As a patient, you have the right to:

  • Access equal medical treatment and accommodations, regardless of race, creed, sex, national origin, religion, or payment source.

  • Receive comprehensive information from your physician about your diagnosis, treatment, procedure, prognosis, risks, and side effects before any procedure.

  • Exercise your rights without fear of discrimination or reprisal.

  • Voice grievances about your treatment or care without consequence.

  • Enjoy personal privacy during your care.

  • Receive care in a safe environment, free from all forms of abuse or harassment.

  • Obtain the care necessary to achieve or maintain your optimal health, including support for end-of-life care if needed.

  • Be cared for by friendly, respectful, and qualified staff who deliver services to the highest standard.

  • Understand the scope of services available, after-hours care provisions, and associated fees.

  • Participate in decisions about your treatment intensity and scope, or have a designated representative act on your behalf if you are unable.

  • Make informed decisions about your care and refuse treatment within legal limits, understanding the medical consequences of refusal.

  • Approve or refuse the release of your medical records, except as required by law or third-party payment contracts.

  • Express grievances, complaints, or suggestions at any time.

  • Access and obtain copies of your medical records.

  • Be informed about the Center’s policies on Advance Directives, Living Wills, and Powers of Attorney.

  • Be informed before any transfer to another facility, ensuring the receiving facility has accepted the transfer.

  • Practice spiritual beliefs and cultural traditions that do not harm or interfere with your medical treatment.

  • Receive interpretation services in compliance with Federal Civil Rights Laws if you are not proficient in English.

  • Have your pain assessed and managed, with education provided to you and your family on pain management roles.

  • Change providers if other qualified providers are available.

  • Have your rights exercised by a court-appointed representative if adjudged incompetent, or by a legal representative designated under state law.

  • Obtain credentialing information about our healthcare professionals.

Your Responsibilities

To support your care and the operations of our Center, we ask that you, as a patient, take on the following responsibilities:

  • Bring your health insurance card.

  • Provide government-issued photo identification.

  • Bring a form of payment, if applicable.

  • Provide documentation of Guardianship or Power of Attorney, if applicable.

  • Be considerate of other patients and staff, helping to manage noise, visitors, and distractions.

  • Respect the property of others and the Center.

  • Clearly communicate whether you understand your treatment plan and expectations.

  • Keep appointments or notify your physician and the Center if you cannot attend.

  • Provide accurate and complete information about your health history, including complaints, illnesses, hospitalizations, and medications.

  • Report unexpected changes in your condition or other health concerns.

  • Follow the Center’s prescribed rules during your stay and treatment, understanding that non-compliance may result in forfeiture of care.

  • Promptly fulfill your financial obligations to the Center.

  • Identify any patient safety concerns.

Advance Directives Policy

All patients have the right to participate in their healthcare decisions, including creating advance directives or designating a power of attorney to make decisions on their behalf when they are unable to do so. Bonneville Surgery Center respects and upholds these rights.

Unlike acute care hospitals, our Center typically performs low-risk procedures. However, no surgery is without risk, and your physician will discuss your procedure’s specifics, including risks, recovery, and post-surgical care.

Our policy, regardless of any advance directives or instructions from a healthcare surrogate or attorney-in-fact, is to initiate resuscitative or stabilizing measures and transfer you to an acute care hospital for further evaluation if an adverse event occurs during your treatment. At the hospital, further treatment or withdrawal of measures will align with your wishes, advance directives, or healthcare power of attorney. Agreeing to this policy does not revoke or invalidate any existing healthcare directive or power of attorney.
