Items to Bring
Please ensure you bring the following items to your appointment at the Bonneville Surgery Center:
Health Insurance Card
Government-Issued Photo Identification
Form of Payment (if applicable)
Guardianship or Power of Attorney Documentation (if applicable)
A list of your current medications
Physician Consultation
Your surgeon will inform you of your arrival time at the Bonneville Surgery Center on the day of your procedure. They will also provide specific medical instructions or restrictions to follow before your surgery.
Pre-Operative Phone Call
A nurse from our Center may contact you one or two days before your procedure to review your health history and ensure you are prepared for your surgery.
Health Changes
If you experience any health changes, such as a cold, fever, or rash, please notify your physician immediately before your scheduled procedure.
Responsible Party Requirement
You must be accompanied by a responsible adult who will drive you home after your procedure and stay with you for the first 24 hours following discharge. This individual must remain at the Center during your entire visit. For patients under 18, the responsible party must be a parent or legal guardian.
Midnight Before Surgery
Food and Drink: Do not eat or drink anything after midnight, including water, coffee, tea, gum, chewing tobacco, mints, or candy, unless otherwise instructed by your physician.
Morning of Surgery
Food and Drink: Refrain from eating or drinking unless specifically directed otherwise by your physician or pre-op nurse.
Medications: If prescribed, take blood pressure or heart medications with a small sip of water. Your physician or pre-op nurse will provide guidance on all other medications.
Clothing: Wear comfortable, loose-fitting clothing that is easy to remove.
Contact Lenses: Bring glasses and a case, and do not wear contact lenses to the Center.
Valuables: Leave all jewelry, valuables, and piercings at home to ensure their safety.
